For GPs
To refer your patient to the Julia Argyrou Endometriosis Centre at Epworth, download our referral form (PDF, 110KB). We also have an RTF version (RTF, 276KB) that can be used with your practice management software. Please email or fax the completed referral form to our endometriosis nurse coordinator, and they will be in touch once they have processed the form.
For patients
Alternatively, if you have endometriosis and want to be referred to our centre, you can download our referral form and take it to your GP.
